The online vendor application process allows you to complete all steps—form submission, logo upload, Health Permit upload, and a 25-word business description—on our website. Once submitted, our team will review your application and follow up with the next steps. No paperwork is required.
2-step Process
Step 1 - Download and fill Vendor and CC Health Permit. Then, Scroll Back Up to Digital Form. Complete the Digital Form below and upload all necessary files, including the Health Permit.

For a traditional option, download the Contra Costa Health Permit and vendor application from our website below, complete both forms and email it with your logo and 25-word business description to Maggie Alinsod at mggalinsod@gmail.com. For information: 510-847-7401
